Your insurance plan may cover nutrition counseling!

Here are the steps to take to find out what is covered.

Rates & Health Insurance

At The Concierge Dietitian, most of our clients have health insurance plans that cover nutrition counseling sessions with our dietitian.

The Concierge Dietitian is in network with Cigna, United Healthcare, Medical Mutual of Ohio, Ohio Healthy, Medicare.

Insurance Verification & Coverage

To schedule your first session using insurance benefits, you’ll be prompted to securely enter your insurance information.

Please note that it is your responsibility to contact your insurance provider and verify your nutrition counseling benefits, coverage, copays, deductibles, coinsurance, referral requirements, prior authorization requirements, and any visit limitations before your appointment. Insurance coverage varies by plan, and benefit information provided by insurance companies is not guaranteed.

As a courtesy, we may assist with submitting claims or obtaining information when possible; however, we cannot guarantee coverage, payment, or reimbursement for services rendered. Any charges not covered by your insurance plan remain your responsibility.

Many insurance plans offer out-of-network benefits, meaning that even if we are not in-network with your plan, your sessions may still qualify for reimbursement. Upon request, we can provide a superbill for you to submit to your insurance company. Please note that payment is due at the time of service for all out-of-network clients.

Please note that our Private Concierge Nutrition Package is not covered by insurance. If your insurance company denies coverage, applies the services to your deductible, determines the services are not medically necessary, or otherwise declines payment for any reason, you are responsible for the full amount billed.

đź“‹ Verifying Your Benefits

Before your first visit, we recommend calling the member services number on the back of your insurance card to verify your coverage for Medical Nutrition Therapy (MNT).

While our team is happy to help you understand your benefits, every insurance plan is unique — some are self-funded or “grandfathered” and may follow different coverage policies.

How to Check Your Insurance Benefits

When you call your insurance company, ask the following questions to confirm your coverage for nutrition counseling:

  1. Does my plan cover Medical Nutrition Therapy (MNT) with CPT codes:
    • 97802 (initial appointment)
    • 97803 (follow-up appointments)
  2. Does my plan provide preventive coverage for nutrition services under the Affordable Care Act?
    • Ask about diagnosis code Z71.3
    • Ask if overweight or obesity are considered preventive
  3. Does my plan also offer medical coverage for MNT?
    • What diagnosis codes are included?
    • Is there a limit on the number of visits?
  4. Do I need to meet a deductible before coverage begins?
  5. What is my copay or coinsurance, if any?
  6. Does my plan cover telehealth appointments?
  7. Do I need a referral or preauthorization from my doctor?
  8. Always ask for a reference number for the call and the representative’s name — this can be helpful later if questions arise.

Need a Referral?

If your plan requires a referral, please have your doctor’s office fax it to 614-647-8431, including all relevant diagnosis codes (such as overweight or obesity).
